A 26-year old lady troubled by a dorsal bump and an excessively wide nose underwent open ultrasonic rhinoplasty and alar base reduction. An ultrasonic technique was used to achieve a smooth contour and make accurate adjustments to the bone. As well as feminising the side profile, tip refinement was achieved using alar contour grafts and a septal extension graft to help shape the nostrils. In addition, an alar base reduction through a small incision at the base of the nostrils to reduce nostril width.
